HOW TO REGISTER There are a variety of ways to register for our conference. Please choose one of the following:
1. Online Internet registration is available 24 hours a day, seven days a week. You will receive an email confirmation once your registration has been received. If you are paying by credit card, we accept Visa, MasterCard, American Express and Discover. This is the easiest way to register.
2. Fax*: Download the Promising Practices 2012 Registration Form (credit card information must be provided). Fax number: 1.916.446.0427 - Attention: Joua Xiong, HEC Conference Registration.
3. Mail*: Download the Promising Practices 2012 Registration Form (credit card information or check must be provided).

4. Purchase order*:

*Faxed, mailed and purchase order registrations may take up to 10 business days to process.
REGISTRATION CANCELLATION POLICY
You may cancel your registration by sending a written cancellation request via e-mail, fax or mail to be received on or before April 1st, 2012. A $50.00 processing fee will be deducted from your original registration rate. After April 1st, 2012, you will not be able to receive a refund of any kind.
You may transfer or assign your paid registration to another participant in the event you cannot attend the conference.
In the event Promising Practices 2012 is cancelled, Health Education Council will refund all conference participants their full registration fee.

SCHOLARSHIPS
The application process is now closed. Notifications will be emailed to scholarship recipients on November 4th, 2011. The scholarship covers the conference registration fee.
