The Healthy Eating, Active Living (HEAL) Collaborative is made up of agencies and community partners committed to improving the health of individuals, families and communities within the 14 counties of the Gold Country Region (Alpine, Amador, Calaveras, El Dorado, Mono, Placer, Tuolumne, Sacramento, San Joaquin, Solano, Stanislaus, Sutter, Yuba and Yolo).

Please don't hesitate to contact us using the e-mail above or by phone (916-556-3344) with any questions regarding membership or HEAL Collaborative activities.  Once we've received your membership form, you will receive a New Member Orientation packet to help familiarize yourself with current efforts and further background about the Collaborative.
